Cho Gue-Sung and Aral Simsir both struck the same post during the shootout, while Edward Chilufya slipped at the crucial moment and failed to convert. Morgan Gibbs-White, Ibrahim Sangare and Neco Williams all held their nerve from the spot to send Forest through.
Vitor Pereira’s side will now meet Porto in the last eight next month, setting aside their domestic struggles to extend their European campaign.
A closer analysis of what happened on the night
Ryan Yates had earlier given Forest control of the tie with a driven effort from 20 yards, after Nicolas Dominguez had drawn them level on aggregate before the interval. Midtjylland responded through Martin Erlic, whose close-range finish forced extra time.
Forest arrived in Denmark trailing from the first leg after Cho’s goal at the City Ground, but they began with urgency and purpose.
Pereira made nine changes following the goalless draw with Fulham, mindful of a looming fixture against Tottenham, yet his side dominated the opening period, registering 11 attempts to their opponents’ solitary effort on target.

Yates came close when he rattled the crossbar following an incisive pass from James McAtee, while Philip Billing cleared off the line amid a scramble in the area.
The breakthrough came five minutes before half-time. Dilane Bakwa delivered a deep cross which Nicola Milenkovic nodded back across goal, allowing Dominguez to stretch and guide a looping header beyond Elias Olafsson.
Yates extended Forest’s advantage seven minutes after the restart with a precise low finish, but Erlic levelled the tie once more when he reacted quickest after Morato had blocked Cho’s initial header, driving the loose ball past Stefan Ortega.
There was late drama in extra time when Yates appeared to have settled the contest with a well-placed header, only for the assistant’s flag to intervene. A VAR check upheld the decision, ensuring the tie would be decided from the penalty spot, where Forest ultimately held their nerve.
